Part I

Antipruritics

Antipruritics, also known as anti-itch drugs are medications that inhibit the itching (Latin: pruritus) that is often associated with sunburns, allergic reactions, eczema, psoriasis, chickenpox, fungal — infections, insect-bites and stings like those from mosquitoes, fleas, and mites, and contact dermatitis and urticaria caused by plants such as poison ivy or stinging nettle.

Topical antipruritics in the form of creams, sprays, ointment, dusting powder, ointment, lotion, paste are often available over the counter.

Creams may contain an antimicrobial preservative, unless the active ingredient or basis has sufficient bactericidal or fungicidal activity. Generally creams are cosmetically more acceptable than ointments, as they are less greasy and easier to apply.

Dusting powders are known to contain one or more active ingredients with or without auxiliary substances. They are intended to be applied to skin for therapeutic, prophylactic or lubricant purposes. Dusting powder intended for large open wounds or severely injured skin should be sterile. Oral anti-itch drugs also exist and are usually prescription drugs.

Part II

Anthralin

Anthralin is a medicine applied to the skin of people with psoriasis. It controls the extra-rapid growth of skin cells. It is not a cortisone, but a strong medicine that can cause the skin to become sore and irritated.

Anthralin does not start to work until it has been used for several weeks. One of the advantages of anthralin over the cortisone ointments is that once the psoriasis goes away, it stays away for a longer period of time.

Mode of action. Anthralin is available as a greasy ointment (Anthraderm) and also as a non-greasy vanishing cream (Dritho, Lasan). When using anthralin, you should follow these directions:

Keep anthralin away from your eyes, it may cause severe eye irritation.

Wash your hands after applying anthralin so that you do not accidentally touch your eyes.

Do not apply anthralin to the skin-fold areas such as the armpits, groin, genital, and rectal regions because severe soreness may occur.

Use very small amounts of anthralin. Apply it to your psoriasis, and gently rub it in. After 30 minutes of contact with your skin, you should wash it off in a bath or shower. You can increase the amount of time the medicine is in contact with your skin up to 1 hour.

If  anthralin irritates or burns, the skin too much, you can wash it off sooner than 30 minutes. If severe irritation occurs, stop to use anthralin and contact the doctor.

How to use. This medication is used for the treatment of long-term psoriasis. It is not meant to be used for a severe outbreak of psoriasis. Do not use this drug if the skin is inflamed or irritated. Follow all doctor and pharmacist directions exactly.  This medication is for use on the scalp and/or skin. Apply this medication using plastic gloves since fingernails may become discolored. Apply thin layer to the psoriasis lesion. Avoid the normal skin or scalp. Rub in well. For the scalp: wash hair first with a shampoo to remove loose scales. Rinse off the shampoo then apply medicine as directed only on the psoriasis area. Avoid contact with the eyes. At the end of the contact period (usually starts as 15-30 minutes per treatment) remove the drug by rinsing/shampooing scalp or washing, taking a bath or shower to remove any drug from skin. Wash hands after using this medicine. Treatment is usually continued until the affected area is entirely clear.

Side effects. Redness or irritation may occur temporarily. If these effects persist or worsen, inform your doctor.  Notify your doctor if you develop a skin rash, burning, inflammation or swelling of the area where the medication is applied. These may be signs of an allergic reaction. This medication may temporarily discolor fingernails or discolor gray/white hair and stain skin and fabrics. If you notice other effects not listed above, contact your doctor or pharmacist.

Precautions. Tell your doctor if you have kidney disease, allergies. Do not allow the drug to come in contact with eyes, mucous membranes (e.g., lips, mouth), or genital area. Caution is  indicated with the applying of Anthralin  on the areas between toes and fingers, on the face or in skin folds. To prevent staining clothing or bed linen, protective dressing may be useful. Anthralin stains clothes permanently.

This medication should be used only if clearly needed during pregnancy. Discuss the risks and benefits with your doctor. It is not known if this medication is excreted into breast milk. Consult your doctor before breast-feeding.

Drug interactions. Tell your doctor of any over-the-counter or prescription medication you may use, especially of: all skin products or shampoos used for psoriasis, corticosteroids (e.g., triamcinolone, amcinonide creams). Do not start or stop any medicine without doctor or pharmacist approval.

Notes. Do not share this medication with others.

Missed dose. If you miss a dose, apply it as soon as remembered. If it is near the time for the next dose, skip the missed dose and resume your usual dosing schedule. Do not «double-up» the dose to catch up. Anthralin turns out to irritate the skin and can make it very red and sore. Careful attention to details of treatment will help you to get the benefits of anthralin treatment while avoiding skin irritation.

Overdose. If overdose is suspected, contact your local poison control center or emergency room immediately. This medicine may be harmful if swallowed. If this product was taken by mouth, contact your local poison control center or emergency room immediately.

Storage. Store at room temperature between 15 to 30 degrees C away from heat and light. Do not store in the bathroom. Discard unused medication after expiration date on the container.
